New client Form
This intake form is used to gather new clients’ information so that we have a better understanding of your unique needs. We tailor all of our care and services to each individual client as we recognize that not everyone’s pregnancy or birthing situation will require the same care needs. The responses you provide will only be shared and viewed within the Rooted Doula Collective team.
How It Works:
When we receive your intake, the admin team will confirm which doulas available around the time you are requesting services. We will then send your intake to the available doulas who will reach out via phone or email to schedule a free consultation. The exact services package, fee, and contract details will be established with the doula you choose to move forward with.
You can expect a response within 3-5 business days.
